The dysfunctional uterine bleeding (DUB) in maturity women is menorrhagia from the endometrium of the uterus due to luteal dysfunction without any organic disease and it sometimes needs the positive management.
Lecturers of Koryo obstetric and gynecology department in Pyongyang medical college of
193 maturity women diagnosed as DUB in Pyongyang Maternity Hospital were recruited on a volunreer basis for our study from January of 2011 to May of 2015.
Subjects were divided into 2 groups such as study group [irregular ripening (IR) group = 45 and irregular shedding (IS) group= 54] and control group (irregular ripening group = 42 and irregular shedding gtoup = 52).
LR2, LR3, SP10 and GB34 were selected. We applied these acupoints for 15~20 min giving strong stimulation with the interval of 5 minutes once per day to study groups. We injected 0.5% progesterone 20 mg per day to control group.
The therapy period is 5 days.
During the treatment we observed the amount of bleeding, the thickness of endometrium and follicle change through vaginal ultrasonography, endometrium (H-E stain) through the microscope. Serum estradiol (E2) and progesterone (P) level were measured.
When we observed the endometrium through vaginal ultrasound with interval of 2 days, the thickness of it was decreased in IR of study group after acupuncture while it was increased in IS of study group and control group (P<0.05).
The growing of follicle was faster in study group than in control group.
Histological change in endometrium: The endometrium was changed from mixed type (secretory phase and proliferating phase) to early proliferating phase in IR and from mixed type (early proliferating phase and secretory phase) to last proliferating phase in IS of study group in 5th day of treatment. But in control group, the endometrium has changed into secretory phase in 5th day of treatment.
The serum E2 level in IR of study group incresed from 52.79±30.95pg/mL (before acupuncture therapy) to 81.16±19.84pg/mL (in 5th day of acupuncture therapy) (P<0.01) while it decresed from 167.14±42.54pg/mL to 114.61±30.26pg/mL in IS of study group, from 53.43±26.82pg/mL to 41.2±25.67pg/mL in IR of control group and from 168.37±58.27pg/mL to 41.06±19.33pg/mL in IS of control group in 5th day of treatment.
The serum P level decreased from 2.96±1.21ng/mL to 0.31±0.11ng/mL in IR and from 5.64±1.47ng/mL to 0.32±0.13ng/mL in IS of study group (P<0.01) in 5th day of acupuncture while it was increased from 2.92±1.14ng/mL to 5.96±1.38ng/mL in IR of control group (P<0.05). In IS of control group, the serum P level had not significant difference before and after treatment.
Through this study we discovered the acupuncture therapy is very effective for dysfunctional uterine bleeding in matunity women by recovering normal hypothalamus-pituitary-overy axis and stabilization of endometriu.
The study on the Koren Traditional Medicine for the treatment of dysfunctional uterine bleeding will be continued.
