Therapy of the chronic apical periodontitis by calcium hydroxide chlorhexidine pasta

Generally speaking, in the conservative dentistry practice, the chronic apical periodontitis is the disease which has the high rate of recurrence, so there have been a lot of studies to improve the therapy, but no success.

Recently, the calcium hydroxide has been introduced for the therapy of the root canal and widely used as the medicament, and now it is the first choosen medicine in the disinfection of the root canal which is infected or not.

Though it has good efficacy in the disinfection of root canals, there are some reports which say the recurrence rate is still high owing to the anatomical characteristics of the root canal and the resistance of bacteria.

Especially, after there was a report which said the recurrence rate of the chronic apical periodontitis was more than 25 percent, they concluded that they couldn't disinfect the infected root canal completely by only calcium hydroxide, so that in some countries they are going to combine calcium hydroxide and antibiotics for the disinfection of the root canal.

Calcium hydroxide chlorhexidine pasta raises the success rate of the therapy of the chronic apical periodontitis by the combination of antibiotics and the previous root canal disinfectant.

In the department of conservative dentistry of Pyongyang Medical College, Kim Il Sung University, they developed the medicament, for the disinfection of the root canal, which is urgently needed in the clinical practice, thus enhancing the efficacy more than previous therapy.

Calcium hydroxide chlorhexidine pasta which is made by combination of calcium hydroxide and 2% chlorexhidine solution was applied to the root canal by H-file or lentulor.

The subjects were the 158 patients who have chronic apical periodontitis. The subjects were divided into three groups; calcium hydroxide chlorhexidine pasta for the study group, calcium hydroxide distilled water pasta for the control 1, and iodine tincture for the control 2.

Followed by opening of the pulp cavity, drainage of pus and the root canal preparation, we did medicament.

According to the improvement of clinical symptom, the results were classified into very good, good, bad and worse.

After 18 months, the efficacy of improving the clinical symptoms of the chronic apical abscess was significantly higher in the study group(96.8%) compared to control 1(88.2%) and control 2(78.3%).

And in the case of chronic apical granuloma, the efficacy of improving the clinical symptoms was significantly higher in the study group(84.6%) compared to control 1(70.0%) and control 2(63.3%).

Through the study, we could conclude that calcium hydroxide chlorhexidine pasta had good efficacy for the treatment of chronic apical periodontitis.

We keep studying to apply calcium hydroxide chlorhexidine pasta to the treatment of chronic apical periodontitis.
